Everytime I go into cover, it will randomly pop me out. It happens about 90% of the time when I use a power, switch weapons, or reload. It will also happen when I lean out of cover to shoot. It will also pop me out randomly, and when I try to go back in, it pops me out almost instantly. This continues four times. This problem has gotten me killed so many times that it's hard to count anymore. I'll go into cover, hit X to reload, then it will pop me out of cover and reload so that I'm completely helpless for a good 3 seconds. Does anyone else have this problem?

It happens to me too, sometimes it's a known issue from level design, changing weapons (especially as a Soldier swapping between my sniper rifle to assault rifle and/or heavy weapon), or Shepard just randomly drifting to the left when I'm trying to issue a squad command.
For me the drift to the left is even noticeable when I save (especially on the Normandy) or when I just loaded a save if my thumb is off the thumb stick.
The worst part is that I've I've not heard of a solution or work-around for these issues yet.
But, do you use a wireless controller by any chance? I ask because I've been wondering if there could some sort of interference issue with wireless controllers.

No, I keep my controller wired, but I can unplug it and it happens the same way whichever way I have it.

This also happens to me too, I was hoping to see if there was a fix somehow, perhaps its cause by the DLC? I currently own all the mission DLC, firepower pack and Alternate Appearance 1&2. Gonna try to reinstall them and see what happens.
edit* the problem seemed to be solved for me atleast after i deleted all the DLC, clear the system cache and redownload the DLC and then patch it.

I don't have any of the DLC and this happens to me. Haha. I thought it was a controller issue but it happens with both of my controllers reliably. One is wired, the other isn't.
