So far ive come across two:
1) in orzammar, when trying to decide who i should have gone with, i approached both spokemen.  I ended up going with Harrowment just because his 'errand' seemed far funner and the town seemed to consider him the better man.  Anyways, im far done with questing in Orzammar but i still have the origonal quest received from the spokeman of the prince in my quest log...i mean, theres nothing i can do as far as completing the quest because i didnt actually accept the quest...and as for the person who told me about the quest but that didnt actually give me the quest - i had no other choice but to kill him when i approached him later....why is this anomaly still here?
2) In Denerim, the mages collective has a quest to go searching for that crazy guy out in the woods (near the Dalish).  I truly do feel terrible about this but i ended up killing him when i went through that area...since i picked up the Dalish before i ventured to Denerim.  I've tried accepting the quest and then turn around to tell the collectives rep. that "Sorry, but the crazy dudes dead." but it wont work.  ...what...tf?
