I got the impression that the reason Fen reacted as he did after Hawke/Fen relations is not because of pain, but because of the flashbacks. And I thought the flashbacks had more to do with the intense emotions he was experiencing during said relations. If it's just fun sexytimes with Isabela, then it's not an issue! No emotions! He loves Hawke! Right? ....That's the version I'm sticking to!
Also, he is the equivalent of a virgin as far as his memory is concerned, yes. But he does say he didn't find anyone to trust since being free, and that's why he didn't get close to anyone. It sounds like he and Isabella just have a fling thing going on.
Maybe I'm reading too much into this.
