Havent' seen this mentioned anywhere and wondering if I am the only one. I have noticed that if you revive fallen companions in battle, sometimes they don't go into attack mode and just walk around with their weapons holstered. You have to manually have them attack...and, then they won't continue to attack unless you keep clicking. This necessitates you having to manually control them the rest of battle. I am not sure how to break them out of it.
Also, sometimes, one of my characters, either Hawk or a companion, will start running away and into a wall...you have to control them to stop it. Quite annoying during a tough battle.
During the unbound scrolls quest in Act 3, during the boss fight in Darktown, Varric was just sitting there during the battle...I was wondering what the hell he was doing when I noticed.
Possible bug - Party members don't attack after being revived in battle?

I've very much seen the same thing. It's very annoying when you're getting your tail beat and your companions are wandering aimlessly around. 
I've also noticed that sometimes, if you stand still, they'll walk back over and assume their group position. That seems to wake them up and they then run into battle.

I've sometimes fixed that revive issue by going back to the menu screen then resuming. About a 50% success rate.

I've noticed this too. Pretty annoying indeed. Let's just hope this is one of the hundred bug fixes.

I've had this problem on a couple of play throughs trying to beat the High Dragon at the bone pit in ACT 3

I've noticed this bug as well. It never lasts long, though.
