What exactly are the five decisions needed to side with a particular group for the achievement/trophy?

The achievement/trophy description says sided with the (X)s five times. What quest/ decision points do they mean exactly? During one of my playthroughs, I tried having a a rogue that cared for the mages but still sided with the templars for the most part, lets just say I didn't get the achievement .
