I don't remember the exact lines but Isabela starts a joke where she asks for the difference between a **** and something else, and says that the **** doesn't have nice boots, where Aveline ends with "Your boots look fine to me"

LOL, you have to remember, he does state that he treats most of the people at the Blooming Rose at his clinic. He knows where they have been and what they have.

As far as I know you only get Rivalry + from Anders and Merrill if you pay for a hooker... while you're romancing them. He's not objecting to you paying for sex. He's objecting to you screwing a hooker while you're "dating" him.
