The dialogue options need to be clearer

I am not a huge fan of the dialogue wheel, but I don't have anything against it either, the only problem I have with it in DA2 is that sometimes Hawke says something completely different than what I intended, and this kinda sucks. Does that happen to anyone else or is it just me?

Nope, just your copy of the game.
It's never something serious, though. Pick the offensive answer, and you offend the other, regardless of if what you said differs from what you picked.
