I just completed Dragon Age: Origins for the third time on Nightmare hoping to get the last of the achievements on the Xbox 360. I manage to accomplish all but one of the achievements and I believe it is a glitch of some kind, was hoping there was some way to resolve the issue without having to play through again.
The achievement in question is "Completionist" and the two endings I did not have before completing this Nightmare playthrough were Alistair slaying the Archdemon and my own character slaying the Archdemon. I have a seperate save in which Morrigan's ritual is completed and another save in which Teryn Loghain slew the Archdemon.
Before completing the game I created a seperate save right before the Landsmeet and then continued on with the game, after defeating the Archdemon Alistair proceeded to finish the Archdemon off since my character romanced him on this playthrough. This would be 3 of 4 endings required for the "Completionist" achievement, unfortunately though at the Post-Coronation Anora spoke in the cinematic as if Alistair was dead, but when I talked to her NPC she acted as if Alistair was alive. Every other character acted as if Alistair was dead, primarily Wynne, and the Epilogue gave me an ending as if Alistair was alive. I believe this is what caused the glitch, but I'll continue just in case.
In order to get the fourth ending necessary to complete the achievement I loaded up my previous save right before the Landsmeet, turned Logain into a Warden so Alistair would leave the party and then proceeded with the game. After defeating the Archdemon I had my character finish her off fully expecting to get the achievement since I would now have four seperate saves each with a different ending...but did not receieve the achievement.
Sorry for the block of text but is there anyway to resolve this issue? Otherwise I will have to go through the game again and take steps to receieve each of the four endings to make certain that I have all four endings on seperate saves - which I am not too inclined to do at the moment.

(1) Performed the Dark Ritual: Everyone lives, Warden delivers killing blow.
(2) Turned down Dark Ritual: Alistair delivers killing blow to Archdemon and dies.
(3) Turned down Dark Ritual: Loghain delivers killing blow to Archdemon and dies.
(4) Turned down Dark Ritual: Warden delivers killing blow to Archdemon and dies.
In your post I see you only mention one 'game' where your Warden kills the Archdemon, but Loghain twice. I think you're still missing one of the Warden's one.

I probably was not too clear in my wall of text so =P
I have three total playthroughs, one where Morrigan's ritual was performed.
Two where Morrigan's ritual was turned down and Teryn Loghain slays the Archdemon.

Afterwards I loaded up a different save with the same character in which instead of supporting Alistair I make Loghain a Warden at the landsmeet and at the end of the game I kill my character - upon which I should have received the achievement but I did not.
I believe it was glitched because at the end of the Alistair dying playthrough during the Epilogue Anora acted as if Alistair was alive and the Epilogue stated that Alistair was a good king who traveled frequently etc..
I would like to know if it is glitched and if there is some way to fix it and/or which ending I still require so I can go back and start another playthrough to get that ending and receive the achievement.

As I recall, if Alistair is not made king and lives the endings are bugged (whether the flags are set correctly I do not profess to know; we'll need a DA2 player to confirm that). It could be glitched as you suspect (in which I would suggest playing through the final battle again).
My original concern is which of (1) to (4) you have done.

The only other way to confirm it would be having a game where you didn't romance Alistair and kill the Archdemon yourself. If I remember correctly...actually maybe not.
EDIT: had a quick search around the site; I couldn't find many having the same problem so it would seem to suggest a glitch rather than a bug. Not that that is any consolation.
EDIT 2: the only glitch regarding the Completionist achievement I could find concerns the Ultimate Sacrifice achievement which didn't pop correctly.

Great! Thank you for searching around and thanks a ton for the help.
I'll play through the final battle a couple of times to see if the ending stops being gliched and realizes that Alistair is indeed dead and hopefully get the achievement.
If it does not work then I'll just play through again and not romance Alistair in the hopes that I get the acheivement.
