Isabella has to use a maker's sigh after the romance to get it to change, I think. There's a bug, as far as I can recall.

yeah. I'm pretty sure you can't cancel the relationship during the conversation after the sex. But once that conversation is done, have Isabella drink a maker's sigh, and her outfit will change immediately.
