I have played through this game numerous times on max graphics settings and never had this problem before. stopped playing it for a few months, now it's nearly unplayable because the graphics "hiccup" every few seconds. any help?

Are you using different video drivers than you were then?
It might be there's some new software you have that's pulling CPU cycles periodically. You could try running ME2 in a window, and keep the Task Manager's (CTRL+ALT+DEL) Processes tab up, sorted with highest CPU use at the top (Click the CPU column, then click it again if it isn't displaying highest first). Play and keep an eye on the task manager, see if anything rises when the hiccups occur.
