Two issues:

1. When not in combat, group members arbitrarily break Hold Position order and one of them starts running after the main character;

2. When in Hold Position order, group members sometimes do not start fighting even if directly hit by a melee attack and having Enemy:Nearest Visible -> Attack orders. This conserns also ranged group members, such as Varric, Merrill and Anders.
