So has anyone confirmed if there are any lasting, major effects from decisions in origins and awakening?  I'm looking for things more significant than finding Alistair drunk in the Hanged Man: quests that are otherwise unavailable, items that only appear under certain decisions from the warden, etc.

I'm also about to finish my last playthrough in origins, and I'm trying to decide if I should have Alistair and Anora rule jointly, or whether to have my Cousland male marry Anora.  any ideas if either of these decisions have real effects in DAII?

If Alistair is king, he apparently has a scene in which he appears in DA2. As far as I am aware, such a scene does not take place if Anora rules alone or marries the Warden.

Other quests that I know of which are affected are Nathaniel's quest in DA2 (only happens if he survives Awakening and has a high enough approval at the end), and a quest about hunting down Amaranthine betrayers which takes place if the Warden-Commander decides to burn Amaranthine and save the Vigil. There's also the Last of his Line quest where the last surviving relative of Lord Pyral Harrowmont is trying to flee to Kal-Sharok and is being hunted by the Carta, if Bhelen is King of Orzammar.

That's all I can remember offhand.

Alistair's didn't really make sense, he went all the way to kirkwall for that?

Nathaniel Howe's cameo was by far the best. It's also hampered by some very specific trigger requirements.
Isabela and Zevran can remember that they had a one-night-stand with one of my Wardens, but somehow it's bugged so I get no acknowledgement from Alistair that he was in love with me, both as king and as a warden at the end of act 2 (with 2 different wardens, obviously). It is mentioned more than once (and in the codex) that "The Hero of Fereldon is now its Queen", if that helps your decision making any. If you like Anora, I say marry her, if not... Alistair IS of Theirin blood...

There's a reference to Bhelan as king of Orzamar, and a related quest with the surviving Harrowmont brother trying to avoid assassination by Bhelan's death squad.

Anything significant? No.

It's honestly a little disappointing. You get a few cameos and one or two quests depending on a few choices, but overall there's no major impact. You could easily play this game without importing a save and be fine. It's nothing like ME -> ME2.

Well that's crap. Do any of these minor quests even have unique items? Are there not even any ramifications from Awakening or Witch Hunt decisions?

I think that it's fair to say that DA2 is a story in the same universe and is largely unrelated to the events in DA1. The events in DA1 are there to add flavor to DA2 as opposed to bearing large, immediate impacts.

[OK fleeing the Blight is very direct ... but once you get to Act 1 most events of DA1 just color the world]

Now in DA3 or later you would imagine that we will get back to those impacts.

I think the only way to get Varric's 4th armor upgrade which is a unique item is during the Finding Nathaniel quest in Act 3. So thats a unique item though it's for Varric only. Witch Hunt  and Golem's dlc currently has no effect DA 2 though there are flags marked in the save for Witch Hunt, maybe future dlc will make use of them.

Apparently allowing Sophia Dryden to live at Warden's Peek allows Hawke to kill her in DA2, and there's supposed to be a mage robe called "the Robe of Hidden Pockets" on her body. I've never seen it, but I want to. >.>

Nothing from 'Witch Hunt' that I've seen, but the Nathaniel Howe cameo requires that he suvives (duh), but also that you spare the Architect (which totally goes against all my good sense).

That appears if you break the curse in DA:O, and the werewolves become humans again.

I also discovered in a "No Compromise" playthrough, where I guess the Dalish were killed and the curse unbroken, that a werewolf representative is in the Dalish camp in Sundermount and will send you on a fetch quest in Act I.

If you side with Zathrien, the Dalish assasin is still there but she appears to be mistakenly assuming some random human is a werewolf instead of one of them that was.

But Awakening's decisions are heavily bugged from what I understand. I never got Nathaniel's cameo, even though he survived. It almost makes me think I should not have imported them. Don't they mess up the Origins romances too? So unless they fix it, it sounds like importing Awakening saves messes up Origins decisions in order to add its own that don't even work.

What do you get if Harrowmont is king? I never saw anything, but maybe I missed it.

In my import I let Zathrian live and killed the werewolves and that scene still happened in my game.

As a whole your decisions in DAO matter less in DA2 than your decisions in ME1 mattered in ME2.
