The issues arise because what the file does is move helms from the helm slot to a hidden "cloak" slot.
You *should* be getting armor set bonuses if you have the latest version of the file.
Actually, though, you won't get modded armor sets. I should probably point that out in the Nexus description. It's because I have to edit the settings for the armor sets to include the cloak slot instead of the helm slot, and I don't touch other people's work.
If you are talking about modded sets, ask the mods' authors to provide a version that sets the armor sets to the slots I mentioned.

You don't get gloves or boots with the mod, no, but you do get the equipment slots unlocked. You should be able to equip the companions with any armor so long as they meet the stat/attribute requirements. In my last playthrough, using DFA, I had Bethany in Aldenon's Vestments (from the mage pack #2 DLC), and Aveline was wearing some sweet boots and helm that I had picked up in some quest or another.
So no, the slots aren't useless.
EDIT: A correction. The restriction is stat-specific, not class (as I originally included in my post).

So stats are not the problem, my characters are level 15-16 by the time talk to Avaline for the first time in act 1 in the keep. I end the game at level 99.5 (100 if I don't skip Bride unbound, Evil Tomes and the chanter's board quests and do all DLC)

You're going to have to give me more information about your other problem. If you're unable to equip an item, the relevant restriction is highlighted in red on the item card. What requirement does it say you're not meeting?

1.No full set bonuses
2. Cannot put any but what was made for them on Merrill, Varric or, Fenris - they vanish if you do.
3. Helmets randomly hide and un hide. (on in one room, off in the next)
4. If you put any helm on Varric, he goes bald.
5. Un recruited companions are randomly in default armor, under clothes.
Not what I am looking for. I want hide helmets to hide them all of the time and, i want ANY companion to wear ANY class and stat appropriate armor. The whole point of a mod to dress them is to make all of the DLC and in game armor useful but this does not do that, still stuck with companion specific armor and still can only use any other armor if it's for your Hawke.
