Cassandra.... maybe not Helena Bonham Carter? Though I honestly can't think of a good replacement.. And the Arishok has to be Javier Bardem, no one else would do.

Meredith could definately be played by Joanna Lumley and she is about the right age.
Betty White is a bit old for Lusine. Is Kim Cattrall to much of a typecasting? hehe
Tom Hardy is a great choice for Anders
John Rhys Meyers for Fenris
Either of the Elthina's you have are good. I might lean to Fionnula.
