Last night, I was seven hours clocked in to a new playthrough when I go about the business of hitting up the Wounded Coast for "Unbidden Rescue", "Tal-Vishoth Caverns" and "Duty". 

The Dark Epiphany quest did not appear.

I am positive I imported my Dragon Age: Origins/Awakening save and spared Avernus in it.

Between the creation of the save file and actually getting to the Wounded Coast I added the Air of Confidence to my game. I have a feeling it may be related.

Anyone else have Dark Epiphany just not show up?
