Can someone please give me a list which companions actually change their outfit and under which circumstances?
I know about Isabela, Merrill and Anders.(Only that they do have the ability to change their outfits but not under which circumstances)

just died in the Haunted quest (Act 3) and suffered no injury after
being revived by Anders with Regroup. Aveline also died in the same
fight and was revived by Anders but unlike Hawke she did have an inury
penalty as normal.
Hawke has no health portion reserve, no injury
symbol and most important, health before death was 233 / 233 and after
death was 233 / 233. Definitely no injury penalty of any kind for Hawke
deaths. Although he can drink inury potions equal to the number of times
he has died recently, there is no point in doing so.

Do you have this problem when Hawke dies without being revived?
I have been playing Patch 1.02 without using any Revive spells or Myrhal's potion, so Hawke only gets resurrected if I win the battle with the surviving party members, and so far, it seems that Hawke gets injuries each time after dying and coming back to life at the end of the battle.
Maybe this injury bug is related to using the Revive/Regroup spell on Hawke?
can't say for 1.02, but in my previous game with 1.01 I never used those spells and the problem with injury occurred every single time

They change outfits when they are romanced, specifically the point when you get the "romance/sex scene". To not get the bug, just don't purchase the upgrades until they are romanced/change outfits.

I might be mistaken, but I thought that if you miss upgrades from previous acts you can no longer purchase them. Is this true?

- the endgame rivalry part is currently 'fixed' in the packages for the next patch so assuming nothing crazy happens that requires it to be cut (or it just breaks again without anyone noticing) it should be in the next patch
- the Amulet part doesn't appear to have had an issue reported for previously so that's in now and hopefully that gets addressed before fixes for the next patch are locked

It will still be a good while before you see another patch. The magnitude of the next patch will have roughly the same (if not bigger) scope as this one so the time from launch until the patch release is probably a decent estimate at this point.

Still can't get Nathaniel's flag to import correctly. Started a new game with import from the end of Awakening with Nathaniel in my party and 100 approval. But DAII still reads that he left the wardens.
It's seems like his approval isn't importing properly if at all, just like all the romances from DA:O.
