@Ailo: Your frustration is understandable, but it's really not the dire picture that you're painting. Reload a save prior to Fools Rush In, and you're done. I haven't encountered a single other bug requiring a reload in my 3-4 times through.

I have also experienced this since Day 1; however, I thought it was because perhaps those scenes are simply more demanding on my GPU

Obviously, a Stamina potion is set to be used, but after exiting out of the Tactics menu and pulling it back up, the = potion has disappeared from the tactics slot.

Here's another very minor bug I've caught.
This happened in each of my playthroughs; after the battle in Ser Varnell's refuge in Act 2, the Qunari corpses have a loot shimmer, but don't actually have any loot.

Edit: just uploaded a video demonstrating this. At 0:21, Hawke loses all of his buffs. At 1:15, Hawke reactivates his sustained modes; by 1:25, when I switch back to Hawke, his buffs are gone again.
It's NOT a Dispel or Debuff mechanic, because Carver - who is right next to Hawke - still has Elemental Weapons and a poison active.

I have 4 seperate save games that I'm playing, and I noticed that only one of my Hawkes has this problem, the other 3 hold the activated modes with no problem.Noctaris wrote...
Happens to me, too, Croc. Just noticed it recently. Poisons and Elemental Weapons drop off of Hawke routinely.
Edit: just uploaded a video demonstrating this. At 0:21, Hawke loses all of his buffs. At 1:15, Hawke reactivates his sustained modes; by 1:25, when I switch back to Hawke, his buffs are gone again.
It's NOT a Dispel or Debuff mechanic, because Carver - who is right next to Hawke - still has Elemental Weapons and a poison active.
