 I'm playing on the hardest difficulty and the only way I find it to be challenging is running out of ammo and the only way I die is from getting stuck. Approach stack of crates, snuggle up before taking cover, pretty often I somehow get on top of these crates or on a wall rail and then I can't get down, and obviously since I can't take cover or move away from enemies, I die. Pretty annoying.

Seems to be an Unreal Engine 3 bug as it also happens in Bulletstorm. Only thing you can do is reload a previous save, or try to wiggle your way off (Generally it's quicker and easier to reload).
