ME1:
1) Kaidan (100% of all missions, except on the few playthroughs where I sent him with the salarians and/or sacrificed him on Virmire; he's super-versatile and my Shepard's LI)
2) Wrex (75% - my standard second squaddie, but swapped out for others when the plot demanded it; he is super-tough and his krogan battlemaster spec is the perfect balance for my Infiltrator and Kaidan)
3) Liara (15% - all of Noveria, for obvious reasons)
4) Tali (6% - the geth series of sidequests)
5) Ashley (3% - mostly around the start of the game, plus Luna on most playthroughs; I really like her character, but Wrex tanks and adds biotics to the mix, so...)
6) Garrus (1% - only when really necessary; I just found him too squishy and prone to death)

ME2:
...This is more difficult to answer, because the loyalty missions make up... what, about 35% of the game? One is obliged to bring each squadmate on their own loyalty mission, so everyone must be used to a certain extent. I think this is actually an excellent mechanic, as it reduces the variables for conversation and forces the player to give each of the squaddies a decent try and thus to really learn their strengths and weaknesses. I honestly didn't give my least-used squaddies from ME1 much of a chance compared to those of ME2. Maybe if I had I would have balanced their use a bit more!
Anyway, the percentage breakdowns below don't include loyalty missions, because each squadmate gets one so they kinda cancel each other out.
1) Garrus (80% - ironically, my least-used squaddie from ME1 becomes my most-used in ME2; my Shepard felt isolated and paranoid about Cerberus, and needed someone she knew she could trust)
2) Thane (45% - once I recruited him after Horizon, he became my standard second squaddie: three-sniper squad FTW!)
3) Tali (25% - similar reasons to Garrus: I knew I could trust her and she had my back)
4) Mordin (15% - used him on any main plot missions due to his Collector research, plus Grunt's loyalty mission)
5) Samara (12% - probably the character I trusted most after my SR1 crew, I took her with me when I needed more biotics in my team)
6) Jack (8% - filled Samara's role until I picked up the Justicar)
7) Zaeed (5% - since you can get him immediately, I don't even have to use Jacob at all past Freedom's Progress; also, tend to use him on Blue Suns missions)
8) Kasumi (4% - ditto for picking her up quickly and ditching Miranda)
9) Legion (3% - love him, but he comes into the game too late to get much use in my squad)
10) Grunt (1% - he's no Wrex in combat)
11) Jacob (1% - only used him at the start of the game; my Shepard's not a fan of Cerberus)
12) Miranda (1% - same as Jacob, even though so many people find her to be the most useful squadmate)
13 ) Morinth (0% - I have never betrayed Samara - not even just to unlock Morinth's loyalty power and then immediately reload; I cannot do that to Samara!).

Wrex on the first one, because he was my favorite character from the first game, and a very good fighter.

On the second one, probably Thane. He is one of my favorite characters, and I happen to play a lot with the Vanguard class, so Thane is good to have along for the occasional Warp explosion. It doesn't hurt that he does a lot of damage with guns, too.

This is who I usually ahve with me for plot reaons in playthroughs. I usually play on veteran so it is easy enough that you don't have to worry too much about who you bring.
In ME2, on Insanity, Miranda would make Nr. 1 since she is the most useful in terms of gameplay IMO.

3. Garrus - used to be on a second spot and faced Saren with me through several playthroughs, but I've discovered the joy of having more then one lift + singularity at my disposal.
4. Wrex - love his commentary, appreciate his ability to take damage, but don't find him as versatile as Kaidan/Liara combo and don't like him as much as Garrus
5. Ashley - the least versatile of all the squadmates, she shoots real good and can take almost as much damage as Wrex, and I do enjoy her comments, but she doesn't allow me as much freedom when playing and unfortunately she dies in all but 2 of my playthroughs.
6. Tali. well at least she's a tech... stopped using her the moment I got quarian ally.

ME2.

I generally rotate squadmates depending on the type of mission, so I'm not sure if I can rate them most to least used, as I use them all at one point or another)..
Zaeed as much as I dislike his personality is the absolute freaking best squadmate to have at your back, based on his fighting abilities :/ Mordin has the best battlefield commentary, ok in a fight, but squishy. Jack and Samara are unparalleled against husks and Jack is almost as good at commentary as Mordin, Miranda and Jacob are decent.., Tali in a direct reversal from my ME1 experience gets to go with me all the time the moment I recruit her - I adore her damn drone and her AI hacking is great when fighting geth/mechs, same applies to legion, Garrus... sigh... he has good abilities, but there's just something with his AI that makes him die like crazy, so I ended up using him the least, Kasumi is very useful against living targets but can occasionally be suicidal, Thane is....like Miri and Jacob - decent, I'm learning to appreciate Grunt a little more, but he's...just bellow Garrus on my used list.

If Jack got some decent armor, she and Zaeed (or Tali) would possibly be my most used team outside of loyalty missions. her wearing straps into battle is the only reason I don't use her as much as I'd like to, breaks immersion too much.

because of fight gameplay and I love them.

Sometimes Tali only for the tech.

Kaidan, Garrus ( the same of Ashley without strong health and immunity, it's more difficult with him in insanity ) are useless to me in fight, Wrex bof bof. I don't take them.

So, I don't use the others.

M2 :

Always Miranda ( one of the most the most useful companion ), often Samara and Zaeed or Garrus and also Mordin. For the best gameplay in fight.

I never use Jacob because he's weak in fight, and Thane because I don't like this guy ( despite he is one of the best squaddies ) , who is the most boring character of Mass effect series.

I like all characters, but Thane, I'm able to swing him in space.

ME1:
All of my playthroughs have been as a Soldier, Vanguard, or an Adept so I always kept a tech specialist to unlock/override containers and access panels. So, Garrus never left my party, and just because she is tough as nails Ashley also almost never left my party. I would occasionally sub Wrex for Ash, but Garrus was a permanent fixture.

ME2:
Miranda has the most useful passive attributes (party bonuses to damage and health) and she can tear down shields, armor, and barriers. So, she was always in my squad (especially for my insanity run). As an added bonus getting to hear Yvonne Strahovski say anything even if it is "All Clear" makes keeping Miranda around a no brainer. I also, just like ME1, tended to keep Garrus in my squad because he is a well rounded fighter. I would occasionally mix things up and use Legion late in the game and Thane because snipers are invaluable especially on the harder difficulty levels. I almost never used Jack, Samara, Kasumi, or Jacob.
