I've noticed in Act 3 , when you get a quest to rescue a companion that has been kidnapped by the rogue mage group lead by Thrask ( this is where u meet up witht he mage from "Act of Mercy"), that Alain has no body, his head is just floating around.  I can't remember the actual quest, but it happens on the wounded coast, you get the location from Templar Karen (i think was his name).  Not sure why all of a sudden Alain's body is not showing up because the first couple tiems I played through the game this wasn't the case, I think it started after the first patch so it might be a bug or glitch in the game, or maybe just me. Anyone else having this issue?

I also had this issue. It was weird, even back in town his body was still missing.
