If you bring Gascard DuPuis with you and confront Quentin, no matter what conversation option you pick to try and dissuade Gascard from Joining Quentin, he joins him anyway.
After you kill both Gascard and Quentin, Gascard's voice is still heard as if he is still alive when he dies.
Both of these are glitches in the quest since you should be able to dissuade him from joining Quentin with the bottom two options which don't work and when he is killed during the battle with Quentin since he cannot be stopped from joining Quentin the game thinks he survived when he did not.

Have not encountered this one. But on one of my playthroughs I told the Templar that Gascard wasn't involved at the end of the Prime Suscpect quest and then when I went to find him during All That Remains it turns out the Templars killed him anyways even though they weren't supposed to.

Act 1 - The crest outside of the Hawke estate (that you get in Act 2) is there during nighttime but during the day it isn't. Don't know if its like this once you get the house, have to wait until I get to that point on this playthrough.

There are chests when you open up that have no items listed but there is a take all option and then you do actually get something. Like the chest in the Blooming Rose in Act 1 that gives you I think its a rune recipe or something, for example.
Also, some issues with the SDTV format still occur. Before the scene would be all warped and stretched now there are some funky issues happening with the black bars on the top and bottom of the screen. Specifically they turn colors and sometimes the scene will bleed down into the bars.
I've been running into the chest problem with nothing appearing in the list pre-patch and post-patch. But it isn't just limited to chests, it also affects corpses and rubble and other things like that.
Also another glitch is when you go into Bartrand's House in Act 2 for Varric's personal quest to confront Bartrand and Varric is fighting alone. There is a chest in there that when opened has NO items in it what so ever. You also don't get anything when you pick the take all option unlike the other chests, corpses, rubble, etc... in the game.

There is a way to have him side with you I did it on my first 2 playthroughs. I am in that Act now, I will try and write down how I do it and send it to ya.

Ok i think I understand now, I had thought that they just made DA2 without checking for import bugs. I'm not really the type of person that reads upo on this sort of thing, even though i am a HUGE dragon age fan. But i probably will just use an import save from origins too get every thing rite. Not to belittle the annoyance of the problem (I havn't played all DLC yet to import from), but the import issues afaik are not present in DA2 itself, it all begins with importing an Origins save into Awakenings where approval ratings etc don't transfer correctly, and the bugs snowball from there. Really, that's what they should have fixed a year or two ago, instead of ploughing on with more DLC. Now the problem is that they'd have to go back and patch Origins/Awakenings to get it to work properly, when they should have been fully patched first time round - both games are still riddled with well-known bugs.
Indeed, and this topic has been addressed several times in this thread. The problem is actually not with DA2, it is with Awakenings and the other DLC. If you go straight from DAO to DA2, everything works. I think the problem is that whatever team they put on Awakenings and on DA2 probably didn't work with each other to the extent that the DAO and DA2 teams did. Until they fix Awakenings, it won't work, because that's where the problem is. It's a garbage in, garbage out problem. What DA2 receives from Awakenings is broken, so it can't fix it.
Whoever made Awakenings such that it didn't import these issues from DAO is who really screwed the pooch here. No one can go back and make your Awakenings playthrough contain stuff it is supposed to contain that it didn't pick up from DAO. If anything, should they ever fix Awakenings (which hopefully they will), you'd probably have to play through Awakenings again with a DAO save to make it work right.
My advice would be to either take it straight from DAO, as has been suggested by Mr. Barret, or take it from Awakenings and decide, once and for all, that Awakenings and DLC decisions are more important than DAO decisions. As for me? The DAO decisions matter a little more.
Or, alternatively, you could buy the game on PC and use one of the several mods/utilities out there to generate a clean save game file with all the appropriate flags and drop it into the PC version. That's something I am increasingly tempted to do.

BTW, the whole you must enter a mode to heal (like Anders' Panacea and the Mages Spirit Healer) but you can't use other spells I severely dislike. I know this won't get changed, but I'm stating that I think it was a bad idea.
How come sometimes the auto save icon appears on screen but there is never an auto save file you can load from?

Yes thats great news because I spent 53 hours and i'm in act 2 where I have yet to notice anything wrong with import decisions.

Unable to activate quest when entering the Hanged Man during the daytime. I have tried it with both having Fenris in my party and not in my party. Quest fails to activate. While on the map of Kirkwall daytime there is a small double arrow quest icon above the Hanged Man. However when highlighting the Hanged Man the little box that usually pops up with a list of quests does not pop up. Quest is incapable of being completed. This is a post patch issue.
Also another issue. If I delete the patch from my system cache and try to load the game up without the patch the game fails to load. I am told my game requires that I have the patch installed on my system for me to continue playing the game. As such the patch broke more than just quests it has also broken the ability to load games after deleting a patch.

It's always triggered properly for me. I've had Bethany taken to the circle twice and each time at the start of Act 2 Merrill talks to me about it.

I always got this quest when Bethany was sent to the circle. In my last playthrough she became a gery warden and didn't get the quest

Did you try to go to the Hanged Man from Lowtown? I've found that the quest never activates if you go directly to the Hanged Man from the world map.

If you open a chest when Varric is alone in the house, I wouldn't expect anything to be in there. It's a figment of Varric's imagination after all...

Apart from the boots of the frozen wastes and the three tomes, I am still unable to use any of my extra content, and as I am now on my 3rd playthough it is slighlty annoying.
