I would say DA2 was a terrible preorder edition for reasons mentioned by the OP. However Atlus does amazing first press/preorder editions usually with full art books and physical soundtrack.

All pre-order bonuses. It kind of devalues a premium CE when you don't get everything with it. It really devalues the CE when there are pre-order bonuses which are frankly better than the sum of extras available in the CE.
Soundtrack CD. By far the best thing a CE can have. NOTE: that's CD, not download, not on a bonus disc but a stand alone CD. Those other stuff are junk.
Good:
Nice packaging. ME2 CE did this very nicely. The tin case, cooler box art and internal packaging of extras.
Seperate Box for game.
Not really that applicable to ME3 but how to play DVDs. BlazBlue UK CE came with one which for that sort of game is very useful.
Full size artbook: Bioshock 2 did this. On the down side it forces a very shelf unfriendly packaging but then again the LP already did that.
Artefact book: Journals, setting detail, colour all this stuff is good.
Story DVD: This is pretty unusual Assassin's Creed: Brotherhood Codex Edition had Lineage which was done for AC2 on disc which was kind of nice. Street Fighter 4 had an hour long exclusive animated film. It was an awful film but the production values seemed pretty good. Props for possibly the most extravagant extra ever
Bonus content: But it really doesn't need to stay exclusive. (see below)
The bad:
Crappy models... What am I meant to do with these?
The Meh:
Making of DVDs. I've got dozens of these I've never watched. I've never really been that excited by any as this kind of material tends to be freely available on the internet.
Mini artbooks: Having bought a few art books I do like them but these tend to be too small to really be interesting. It's something done better in a CE strategy guide because at least it's full side. While usually inoffensive sometimes they can be spoilerific. *looks at ME2*
Super-Limited-Ultra-Legendary-Premium-Editions: I've been hyped up enough to go for this a couple of times but rarely do the extras justify the price tag and often they're kind of junk. I'm running short of space to store this stuff let alone display it even when it is good. Halo: Reach Legendary Noble Squad statue has to get props as the best extra but finding some where to store the damn crate it came in, not so good.
The PLEASE PLEASE STOP DOING THIS! NO, REALLY. IT'S VERY ANNOYING:
Exclusive DLC. It's incredibly annoying to get a game and not have access to all the content even if you are willing to pay for it. First of all a CE should have all the pre-order bonus. It's frankly a joke that you can get pre-order bonuses better than CE content and be expected to pay £10-£20 more for the CE with that being the main feature. Second there is no point keeping the anything exclusive indefinitely. Timed exclusive/ early access is great but really, it's not some how devalued by other people being able to buy it.
Outright best special edition:
Assassin's Creed 2: Black Edition
The statue was awesome and it gave early access to some DLC bonus areas was nice. (included all pre-order bonuses)
The worst:
Dragon Age: Origins UK CE
When "collectors plastic case" (ie, a normal case) is a special feature something has gone very wrong. I really don't know how this CE managed to be so bad but all involved should be ashamed.

Also exclusive DLC content such as weapons or armor that will NOT be available to purchase some time later or anywhere else, as in exclusive to the CE only.

Agreed. Adding to the DLC stuff, even if I get the CE, I hate to get additional stuff day 1 that either should be in the game in the first place, or isn't really part of the actual game. When I buy a game, I want my dvd to contain the game in its entirety.javierabegazo wrote...

More or less agree. Personally, I don't like all that much very big packagings, it's cool at first, but it makes for an expensive package with something of no interest to me. I like to get additional stuff like in the Oblivion CE, a map, a pocket guide to the Empire and a Septim, I found all this neat, but mini statues are worthless to me. I love art books, but statues (which is quite popular nowadays) is a no-no to me. I want something that expands the game somehow, and a statue doesn't do much, unlike a map or little (as in little) trinkets. Behind the scenes stuff is definitely great, although sometimes it seems to be the same thing we saw in vids hyping us the game. [*][*]X-Factor - This is always a wild card and can be things like a sculpture, to a model, to special tokens such as rings, or medallions, maps, you get the idea.[*][b]Special Packaging - this is a very important feature that I feel many people over look. It's really wonderful to sit and just look and appreciate the level of quality that goes into decent Collectors editions packaging. Not simple plastic with a cover slip inside, it's more than your average bear. It's special, of some kind, whether it's metal, tin, or even special embossed glossy shiny covering on paper, it's nice to have special packaging.
[*]Otherwise, I don't think there's something in particular that really should be in a CE, I'm pretty open as long as it actually is worth something to me.
