There are four side quests listed at the Dragon Age Wiki that I've not been able to acquire. I know that there are some side quests that, by virtue of decisions one has made earlier in the game, will not be presented. But this seems to be different as they are minor quests not dependent on anything else in the game. Not sure if they are bugged or what. The quests are:
Elves at Large
Hometown Breed
Ladies' Lights Out
The Lowdown
For Elves at Large, I have not been able to click on the Chanter's Board in Act II at all. I wonder if having the Exiled Prince DLC has bugged this one. The others are quests you get whilst travelling around certain areas at night. Take for example Hometown Breed. I enter Lowtown at night, get a remark by Merrill saying someone should clean up the gangs preying on people at night (something or other), but then there are no gangs to be found after exploring the entire area a few times. These are really only minor bugs, but I wanted to be sure I had done everything before progressing on to the Demands of the Qun main plot quest.

There was a lone bandit hiding up by Gamlen's house that wouldn't initiate combat until I climbed up the steps. Once he was dispatched, the other Dog Lords spawned like they were supposed to. So that's Hometown Breed taken care of. Still having issues with the others, however.

"Elves at large", as far as I can remember requires you to have been favouring the Templars.
It´s akin to "Hunt down three apostates" and "The Underground Railroad" questlines.

Just a quick note - when doing the Ladies' Lights Out quest, I encountered an error in which one of the groups wouldn't spawn. All I had to do was leave the area and come back and run around the zone again, which spawned the final group.
Common sense for most, but I hope this post is helpful for those who Google first and retry only as a last resort.

no need to go out and leave the area. did you try to go up the stairs and open a room?
