Ok my character is not in a relationship with Zev on this playthrough but Isabella still wants to include him and he leaves it up to me.
I thought he only did that if we were in a relationship?

No, he doesn't go where he's not wanted, in that case. Can you imagine the raeg if a Warden could end up in a threesome they didn't explicitly agree to? Zev will always ask the Warden for a go-ahead, unless Alistair is also invited, in which case he declines entirely.
