Charge is a little impote... uhhh, let's just say it doesn't work sometimes.

I understand what it requires.  It can only be used when you can walk to the person on foot.  But sometimes they're just a dozen feet down the hallway and it doesn't work, or they're standing right next to me, or just on 1 'stair' up.  Usually it works, but sometimes it doesn't (I'd say 3/10 times it doesn't).  I've already posted this suggestion for improvement for the ME3 post, but I was wondering if anyone else had this issue, had suggestions, had more info on how charge works, or just wanted to post.

Yeah, sometimes it doesn't trigger properly. Annoying as hell.

Yep, sometimes you click the icon and nothing happens (no 'Can't get a lock!' cry, even). That's not the worst part of it for me, though; sometimes (in specific locations, I'd rather say) the game crashes when I'm trying to charge, and the XP > Win7 upgrade didn't seem to fix it. Well, I just avoid charging in these places. ><

From personal experience;

Charging targets that are on angled surfaces will increase the chance of "error charge". A good example of this is Jack's recruitment mission. Charging up a level works 99% of the time, but this map has a lengthy angled ramp running between the lower and upper 2nd floor. I had considerable "error charges" when targeting enemies on the ramp. Stairs seems not to be an issue however.

Similarly, charging an enemy that is "Jumping over" cover will lead to being stuck on top of nearby structures or "into" a target. Specific area & maps are also at risk of power failure (not just charge); Doorways for instance. Again , from personal experience , Jack's recruitment mission and the Dantius towers were noticeably at risk.

Sometime the "charge" ability is bugged at the moment of spawn. And if you "equipped' it to your "quick power" bar , it will continue to be bugged until you "re-equip" it.

That said it could be entirely random and I'm seeing patterns where there are none.

One trick is to keep some space clear behind Shepard before Charging. If your back is to a wall, you might have trouble Charging.

Also you don't seem to be able to charge into different "rooms." Each collector platform connected or not is considered like a separate room(don;t know the tech terms) and you can't charge between them. In Kasumi's loyalty quest the hallway with the first YMIR you face. It has two parallel hallways, each one is separate enough that you can't charge between them even though there are parts where there is no gap between them, you have to physically step to the other corridor before you can charge.(this got me killed many times). All of which leads to frustration. I don't mind getting killed because I screw up, I do it often and I am fine with it. But can;t charge there irritates me, just like the countless times I get killed because I use a biotic power and instead of ccing them it just makes them glow blue and they continue to unload on me. .

It can be very annoying, you do get used to it to an extent. I would actually prefer if a failed charge triggered cooldown and at least restored your shields and other effects.

Try to exit cover first before charging. Also, you have to make sure your target is reachable by foot and not on a different 'platform'.
