I get this too. background effects will pop above the cutscene letterboxing. Fire, smoke, clouds, lighting will ignore what render layer its on and show up on either the top letterbox or the bottom.
I could see this stray beam of blue light during a cutscene in the Deep Roads. It was the one where the crew found the entrance to the Primeval Thaig, I believe, and I could see a diagonal blue light crossing under where the two human excavators were standing in the background.
Another case of the broken letterbox would be during the conversation between Varric and Cassandra after the end of Act 1. The top of Cassandra's head was just barely cropped off by the letterboxing, and somehow the top of her head would duplicate directly above where her head was cropped, with jaggy edges. There would be two of these duplications that went all the way to the edge of the screen.
I really appreciate what the team is doing for fixing these graphical glitches! Before the patch, cutscenes were frequently interrupted with the character models 'flickering' as if they were trying to adjust or decide if the game was in widescreen or not. I'm glad you guys fixed this, among other various things! I hope eventually the game will reach its intended level of 'finished'!
