Sadly, in our case, we seem to have more chance of seeing Lord Lucan on This Is Your Life than ever getting a result on an 'ETA'.
Confirmation that a patch is in the works would be nice though!

In all honesty the PC patch was extremely light and took care of some graphical issues along with balance. As I recall most things must be submitted to Microsoft before they can be distributed so they are most likely attempting to get the most 'bang for the buck' when they release a patch.

while i was working my way up the keep i walked past a big tree that was in a hallway on the wall
was lookin around the room when i went to the tree there was a glitched hole in the wall
and i was able to walk FOREVER in blackness when i went through the wall
i was able to get back thru to the map where i went threw ..but i was a pretty bad glitch
glitch hole threw a tree on the wall ..not groovy

After talking to Irving and getting permission for Dagna to study at the circle, she was at both places.. Which was odd.. But not game breaking..
Corpses routinely take up to a minute to become lootable after being killed.. Extremely annoying, especially when you kill something that has a quest item you need to loot.
Several quests 'loop' for instance in the sloth demons domain I killed flemeth, by beheading her no less and then the entire script looped back to her talking to morrgan again, but she was headless (Which was admittedly pretty funny)

It is just disheartening that there was time to complete DLC for the initial release.. But not properly debug the core game.. I've ran into all of these except Dagna, haven't got there yet.
Another thing they need to add is a way to disable combat voices. If I hear "Can I get you a ladder so you'll get off my back" too much more, I'm going to ship the game back to Bioware HQ in small bite size pieces.
I really miss when developers had to make sure console games were bug free or watch their game fail, before they were given the ability to be lazy due to the ability to patch like a PC. "Hey, just shove it out. We can patch it later on."
When you add that to the usage of a third party ( Edge of Reality )to do optimizations only to find they don't have a clue what they're doing and make the game look like it's running on a mix of low and medium PC settings, I'm really regretting not getting the PC version. At least PC users have the community modders to fix stuff for them.
I'll definitely be waiting out ME2 instead of buying it because it's Bioware, which was a main reason for my buying DA:O. Whether it's due to Bioware or EA, the quality control has definitely gone downhill fast.

1) The memory leak: Playing for several hours will eventually cause the game to take a minute or two to get you out of combat and spawn loot on corpses(and yes I tried playing from the HDD which helps but does not stop this from happening).
2) Voiced dialog cutting out during cut-scenes: Some conversations drop voiced dialog, sometimes mid-sentence, and no skipping thru dialog is not the cause. The only conversation for me that repeats this everytime is when speaking to the Spirit of the Forest I will get the beginning and end of her speech but it will drop out in the middle right as she is speaking about Z's children.
3) Besides those two I've had a few occasional glitches here and there but nothing re-peatable( Tamlen killing the Blight Bear in the Dalish Origin and it sticking to him as he walked around was funny).
4) Minor graphic glitch with water in the Kocari wilds, next to the tree with the hanging soldiers there is a small pond with one corner where the water texture overlaps part of the land and actually flutters back and forth, but I guess that is what happens when you use a flat texture to represent the water surface(something I thought game developers had learned years ago).

He's probably listing his %age of the world travelled which is also listed in your stats. I'm having the same problem of this achievement not unlocking. In my second playthrough I followed the Prima Guide so I KNOW I went everywhere in the game.

Yeah, I know where it unlocks and that it's dependant on where you've visited. not percentage, just saying the percentage as a rough guide to how much I had travelled.
Got it earlier though.
Turned out that I missed the Orzammar Chantry and either K's/D's hideout in Denerim.
Kinda funny how only 2 places that aren't any large part of the game could make the difference.

Just for an achievment because I was being lazy I brought a warrior from level 4 to 25 in about 20 minues of spamming X and A buttons to duncan

The loot-lag issue to me is the most irritating by far, especially in the orzammar tournment when you're trying to get either Harrowment/Bhelen to like you and you bend over backwards for them. Although having balance would be pretty awesome too I guess.

-Loot detection is to slow, i mean really slow.
-Sometimes the talking wont occur (but the lip syncing works), and i as a person actually like to hear what they have to say since i cant find any subtitle in this game. Either fix this or add subtitle.
Other than this its just minor stuff:
-Nice to see quality color coding on items, but what about item types? its a little bit annoying to scroll down to see if the item got set bonus in a tiny text. At least highlight the set bonus or something.
-The popular level glitch. Nothing major since people can just ignore it.
-Animation stuckness. People can get stuck in one animation, like the head is almost twisted and kids just hovering in the air.
-A better quest helper. In some quests you will just run all over the place to find a quest arrow since the quest dont give you any direction or clues.
And, is there any sticky thread of "report bugs" topic somewhere? I would like to see if the develepors take time to hear us, the consumer.

the loot bug is also very anoying and effects more than just the loot. abilities like the reavers lvl 1 skill get screwed over by it too. Very irritating. I guess play testers never looked at their stats or dmg using daggers, and never played for more than an hour or so at a time? o_O

The funny thing is until now I was the one guy being an **** when people would talk about buying "Backed Up" games, I'd flag the posts on CL when I saw them, and I would even lecture friends who would torrent games (I have 2 playing the PC version of DA that didn't buy it) and what do I listen to daily now? My friends rubbing in my face that their version works fine, yet the one of us that actually PAID FOR THE GAME can't even get so much as an answer from Bioware or EA! It's actually funny it's so ludicrous.
I know for a fact I'm not picking ME2 up when it comes out now, why would I willingly subject myself to this again? Sp goes the old saying, fool me once shame on you, fool me twice shame on me. I've learned my lesson, I was bummed out when I heard Bioware may shut down, now I see there's no reason to really be worried about it, the Bioware I loved is already dead.

Yep, I'm seriously considering selling my 360 copy and just aquiring the PC version via google.

one of the bugs is related to a power you gain from the DLC as a mage. When you use it it becomes a permanent status effect on your character even though it only supposed to effect enemies.
The other is the warden commanders armour. It is supposed to give you a bonus if you wear the full set. except it doesn't give a bonus.
I haven't encountered any of the game breaking bugs yet and haven't played as a rogue yet so haven't noticed the dexterity bug.
But I do hope they bring a patch out soon for all the bugs mentioned on this thread. I don't want to be 50 hours into a character only to suddenly get effected by one of the bugs
