EDIT: Patch installed, and I seem to have traded a major problem out for a minor one. Authorization issues now seem to be a thing of the past, and for this I am grateful. However, area load times have increased dramatically.

As Sami Jo says the test group (and Bioware staff) are still working through the patch trying to find the problems - everyone's trying to be as through as possible so that the final patch, when released, causes no other issues. We are diligently working on finding a solution to this issue. Unfortunately we have very limited information due to the small sample of users not only play DAO but that are also actively posting on BSN. Currently we're trying a new tactic as to figuring out the exact cause of the issue - assuming it works out we'll have a live version as soon as possible.

Bug! Apparently there is a problem where this quest doesn't trigger. A
possible reason for this is that if you didn't import an Awakening saved
game with Nathanial "in your party" at the end, then he is considered
dead even if he survived Vigils Keep. There is a way to work around this
