Is it actually equally distant at all points in the Fade or is that just a bit of false info in the Codex?

Really? It seemed way closer than usual in that Fade section where you have to kill "Duncan" and the other two demons in disguise.

It is not equidistant. It is just always on the horizon, but some places it may appear closer than others. No matter which direction you look, while in the Fade, you can always see the Black City. However, if you ever were to walk in a direction towards the Black CIty, it wouldn't get any closer to you.

I'm not sure that we can be certain about distances in the Fade.

No matter how close the Black City appears to you. You can never come any closer than where you are.

Not in Awakening. Look across the lake behind the Manor in the Blackmarsh Undying and you'll see a very up close view of TBC.
