Eh, I wouldn't think they'll spend the extra resources just to show his healing scars as the game goes on. It would probably stay the same model.

I was thinking more on the lines of Garrus at some point taking the bandage off and going "Wow, that healed fast!" and then keeping it off for the rest of the game. But hey, whatever works! He looks great anyway.

It looked like the concept art had the bandage but the 3D in-game model didn't have it.

I think they're both 3D Models. If you look somewhere around 6:35, you first see the model without the bandage and then immediately after one with the bandage on. Could be Garrus' awesomeness preventing me from seeing clearly.
