I was wondering if this has happened to anyone.

I recruited Zaheed, Jack, Archangel, and the Krogan.  I also collected all the dog tags. I also did plenty of upgrades. I had been asked by Jacob and Miranda to do the loyalty missions.  My trophies say that I have done these things, but when I load my saved game, I only have Miranda, Jacob, and Solis. 

Any suggestions?
