I am unable to drag the higher levels of herbalism out from the skill book beyond the first one, and the first one only has the same 3 things on it that it had when I only knew the first level (I have all 4 now).  How do I get to make other things?

You have to buy additional recipes from various vendors around Ferelden. They're not very clearly denoted compared to other mundane items, you can typically ID them because they'll be listed near the top of the list before weapons/armor and cost a few gp and up. Health pot recipes are bought from Dalish Camp and Mana pot recipes from the mage shop in Denerim, possibly Mage's circle tower but not sure about that.

Oh I see. The Herbalism skill says nothing about having to buy recipes...

I would buy as many of your starter recipes as possible from the quarter master at Ostagar, they are extremely cheap from him, as is the first 10 slot bag upgrade.

The first two backpacks, actually. Buy one before going into the wilds, and one after you return from them.
