I can confirm that I did in fact receive the Belt of the Chastened

Judging by my saves, I picked it up late in act 1, somewhere in one of the following quests (as mage)
Bone Pit
Bait and Switch
Isabella's Contact
Shepherding Wolves
Act of Mercy
Enemies Among Us

Maybe someone else can pinpoint it better than that.

Belt of the Chastened drops from the Mature Dragon at he end of the Bone Pit in Act 1.

Looks like overgrown Glandivalis, in my opinion. But a lot of people seem to like the model. 

That's because Glandivalis recycles the 1h Vigilance model from Awakening
