ACT 1
Intro - The Illusive Man explains why he needs to stop Shepard, and that the Reapers are coming
Trial - Some epic speeches, maybe a chance for new players to catch up. Takes place in the Seattle/Vancouver city
Tutorial Level - Go with James Sanders and the VS, both present at the trial, to get to the Normandy, with Joker. Some epic cinematic bits are vital here. Get out before the Reapers shut the Charon Relay and go to Arcturus.
Arcturus - 'I told you so' with the council. Meet with Hackett and Anderson, and discuss how to retake Earth, and meet up with the rest of the ME1 team that has survived, since this games isn't really about recruiting people as such.
ACT 2 Part 1 - the next 4 missions are non linear
Sur'Kesh - We know that this is going to be in, and that Cerberus are involved. I'd like it to be about the Krogans finding out about the second genophage and threatening war. Shepard helps the Salarians find a solution, but then gets ambushed by a Cerberus team led by Kai Leng (who doesn't get killed yet) trying to put it into action. Defeats the Cerberus team, and is sent to deliver a modified genophage or a cure to Tuchanka, which happens later in the game. Recruits Mordin.
Rannoch - Goes on a peacekeeping mission to stop the Quarian/Geth war, we know this is going to happen. Would be nice if both sides initially try to kill him, thinking that he's helping the other. Either tells the Geth to let the Quarians stay or tells the Quarians to find a new home, with species loyalty depending accordingly. Recruits Legion. Of course, gets ambushed by Cerberus at the end, but Shepard, the Quarians and the Geth all work to take them down, as a nice touch.
Kahje - Only available if Thane survived ME2. You visit Thane during his last days, and find some sort of closure with him. Of course, Cerberus are always after you, and so you get ambushed during your downtime. Thane acts as a temporary squad member, but then gets mortally wounded in a battle with Kai Leng, who still doesn't get killed. You are sad, but the drell and hanar offer to help you against the Reapers.
Rachni Homeworld - The Reapers left some sort of huskifying device on the homeworld, and the Queen asks for your help with her children all being huskified. Only available if you saved the Rachni Queen, of course. You get Rachni loyalty if you do the mission.
Act Progression - Cerberus HQ - You get intel about the location of the Cerberus HQ. Take Jack, if she's alive, to help you destroy it. Find Miranda and Jacob, who, if still alive, offer to make your job easier by acting as a sort of infiltration team. Epic boss battle with Kai Leng. Plant a bomb. Find the Illusive Man's office, and confront him. A choice between saving him or breaking the glass behind him and sending him into that star. Then run like ****. Jack gets caught up and is too slow, Shepard has to abandon her to die in the blast. Recruit Miranda and Jacob
ACT 2 Part 2 - With Cerberus out of the way, you can now focus solely on the Reapers. They've started spreading their wings and sending a few out to clear the path for the next few homeworlds.
Thessia - A timed mission. Get as many civilians as you can to evacuation points and get them out of the system before the Reapers arrive. When they do, trap them and blow up the Mass Relay. Your species loyalty depends on how quick you are, and therefore how many you saved. Samara/Morinth makes a large cameo.
Palaven - A massive trap mission. Lure them into the system, then use all of the Turian military to take them down, lots of big space battles, thanix cannons, lava and radiation should be involved. Learn some more about the Reapers and how you could stop them.
Tuchanka - Go to deliver whatever the Salarians have given you. Half way through, you get ambushed by one or two Reapers, but no worry, lure one into a Thresher Maw nest to take it down. Board a live Reaper and try to analyse info about it. Find out that it's made of Prothean material, have some **** crazy visions, and find out that you need to go to the relics at Mars to find a way to kill the Reapers. By the time you have left, the other Reapers will have been eliminated, at the cost of a few thousand lives, and you can deliver the Salarian's package and leave. Some extent of Krogan loyalty gained, and Grunt recruited for your squad.
After this, there is a countdown clock of sorts, like the one after the Reaper IFF, before you have to enter the Charon Relay
ACT 3:
Entering the Charon Relay - whilst some Reapers have opened the relay to go in/out, take them out and re-enter the relay before it closes. Some Space Combat would be nice here.
Mars - land on the already-Reaped Mars and go to the Prothean relics. The visions of how you can defeat the Reapers becomes incredibly clear - you have to find a way to reverse-indoctrinate them to have any chance of stopping them, or at least giving the races a chance to kill them.
The Final Battle - **** happens. I honestly don't know. A load of people will die, the odd race might get wiped out, the Rachni will come swarming in, the Geth will lure some out to the supernova-ing Haestrom star, maybe or maybe not wiping out either them or the Quarians and you'll find a way to reverse-indoctrinate the Reapers. The final boss battle should be aboard Harbinger, against a huskified version of the Virmire Survivor, that would be epic.

If you want my theory, not just the facts (sort of), we'll see a lot of Cerberus, since unlike the Reapers, they are an easy enemy to write, and in Mars, or perhaps another location, we will have a great revelation about the nature and the origins of the Reapers. Maybe some surreal visions as well.
I also think that the Citadel will be important in it's own regards. The unexplored core was a nice hint in ME1 and I think that there's a lot more to learn about it.

I still would like to see some place in dark space though. A plot mission there would be nice. Maybe the Dark Citadel? I love this theory hahahaha.

The dark space is pretty much the place what you see at the end of ME2. They "wake up" and start moving. My guess is that the relay connecting to the Citadel is somewhere there.
The point of the Citadel Relay was to offer instant transportation to the Reapers. It's possible that we didn't see the Relay in the cutscene, though. And that opens up a lot of possibilites. Could the Reapers have a base? And how would they use it?

I wonder what the factor x will be of defeating or driving the reapers out of the galaxy, cause i doubt it will be simple naval power and gathering or allies. Didn't work with Sovereign so well.
You should add Khar'Shan to that list, or some other batarian main place, cause i think, even through side quest there will be an option to help them and get their help in return or leave them to their doom.
Concerning the Rachni homeworld, i think it's a bonus location for those that saved the queen, but not necessary. Thiks of it as a side mission, that can make the ending a little better, if you decide to go for it.

Nice list, but couple of things:
You should add Khar'Shan or some other batarian place, where you can make decisions that affect that races survival. Paragon's get batarians fleets as a help, renegade can doom the race to oblivion based on their history or whatever or something like that. Also batarian squadmate for those who want it.
If you have hanar and drell as help(like they can help much) you should also get the elcor. I think volus come with the turians naturally.
I doubt we will se so many returning permanent squadmates, they will most likely be new most of them.

And then the ending is the Reapers realizing what you're going to do and they all race to the Citadel to stop the allies from doing so. But then again, if a mass relay is enough to destroy an entire system, wouldn't destroying the Citadel consume the entire galaxy and end everything within it?

The Citadel is a pivotal plot point of the trilogy. It has to be featured again, though under what capacity remains to be seen.Empiro wrote...
