So Isabera is more useful in a fight on my second playthrough than my first, but I still seem to be building her wrong. People talk about how she's very survivable and even capable as a tank. How does I do this? My Isabera dies very quickly.

Well, the problem is that you're using Isabera and not Isabela. A common mistake, as many underestimate the power of the 'L' over the 'R'.
But seriously though, focusing on constitution and dexterity will make Isabela quite the formidable melee fighter. Try to find skills that increase her defense/evade/etc as well, add some defense runes and I could see her tanking to an extent. The tricky part is that tank-Bela won't be able to open locks, since the points you would put into cunning are going into constitution.

which difficulty level are you playing on?
On nightmare, I have tried bringing her along on boss fights in order to take advantage of -50% damage resistance from Below the Waterline, but it is almost impossible for me to keep her alive very long, even though I don't use tactics and micromanage every party member. I can keep her alive, but only if she stands far away and doesn't contribute anything significant to the battle.

As a two weapon rogue, the important attributes are dex for hits and cunning for defense. Willpower is needed to make more of the deadlier attacks. However I have never had to increase strength or constitution beyond 14 and 12 seems adequate. Max out the two weapon skills for killing and get the various skills that allow her to evade and avoid attacks. And find the various special armor for her that are available throughout the game and enhance them with runes. However evade and avoid are her main defenses.

I'm playing on Nightmare, yeah. I forced myself to do my whole first playthrough on Nightmare-- stupid. Easier now, though.
My Isabera is fairly survivable, but not by virtue of being a tank-- I put a lot into the stealth tree for her to drop aggro. And her special tree has some nice defense buffs. However if her abilities are on cooldown and I can't get Aveline to Taunt or whatever, she can drop pretty fast anyway.
About the Scoundrel tree, my companions always seem to get interrupted to death when I try to escape danger with Back-To-Back. I haven't tried out Armistice and Goad much, but their range seems a bit limited. (I assume that means enemies outside of the specified range will continue to target an Armisticed character?)
Since she dies so quickly when attention is on her, I've never really felt like using Across the Bow was a very good idea.
I never thought to go Dex/Con, though, it seems like Cunning would be mandatory for a rogue. Hm.

Goad works well, just have the tank near by
IsabeLa works well if you keep her attacking targets attacking others

Hm, thanks. I had her set to attack the target of Hawke at the end of her tactics instead.
