In my first playthrough Merrill showed up at my f!Hawke's mansion in Act 2 and the entire dialog was romance related (meaning the only options I had were hearts or broken hearts.  I had never chosen a flirtatious response in any conversation with her, but I was diplomatic in all my replied.  In my current playthrough as a m!Hawke, in Act 1 the same thing happened.  

Is it possible to have that happen with Fenris or Isabela and if so, how?  Any information about this would be greatly appreciated.

It's a bug. Merrill isn't supposed to show up asking Hawke to cleanse her taint unless Hawke initiates by hitting a heart. There seems to be an unmarked heart in some conversation(s) somewhere, occasionally folks have reported seeing this bug. It doesn't appear to happen with anyone else.

Isabela and Anders will attempt to initiate romance if they find you sufficiently receptive.

Merrill and Fenris require you to flirt with them at least once.

Thanks for the replies. So Anders is the only one who will bring up the romance even if you don't flirt, eh? (Minus the Merrill bug I got on my first playthrough). That's interesting. I guess I'll know for sure, at least on f!Hawkes in my next playthrough since I'm going to focus on Sebastian and not flirt with anyone else.
