IMO they should add FF as a separate option for all difficulty levels, preferably with an achievement.

Nay -- it will only serve to point out frustrating squad AI shortcomings.  I would much rather have difficulty increased by enemy tactics:
  • Have enemies actively flank, as mentioned.
  • Those hulking Cerberus troopers and husks?  Have them rush Shep like the Krogan in ME.
  • Throw out some effective NPC snipers, also like in ME.

They should make it so all enemies on Insanity have barrier, and change barrier so it can only be removed with biotics before weapons will have any effect. Give all the CoD-soldier-Sheps a taste of what ME2 played like for adepts.

No to them being able to harm you with shooting, that wouldn't be fun if you're playing as a Vanguard/Sentinel, and redundant for the other classes who hide in cover like the pansies they are

I wouldn't mind it being on, since I usually send my squadies to cover my flanks and stay out of my line of fire. Even when I don't its rare for them to get in the way even with their AI.

Otherwise it would just be an annoyance more than anything.

No. This is an abhorrent idea. The squadmates are already flimsy enough without me worrying about an errant spray of my AR mowing them down.

I changed the ini value that allowed for friendly fire in ME1 and played through that.
I'd totally do so for 2 if I could find it ^^

So for me: YAY! Though, for the sake of those who don't like, only if it's optional =P

No, for obvious reasons of the squadmates doing silly things.

HOWEVER, maybe heavy weapon damage radius/ grenades/ aoe powers should all be able to deal damage to Shepard/squad if they are caught in the area of that attack.
I know certain things already do that - cain for instance, but other things like area incinerate, warp bombs, avalanche (etc) should have the potential to harm Shepard if he/she is in range.

I'm not certain on that - could have some problems, but might be interesting.

I'd say yay. I'm very careful with my shooting already, I'd like it to matter. To be punished if I make mistakes.

Since the squad is borderline useless on Insanity anyway, why not?

Yay.

Also, squadmate deaths on Insanity should be permanent as well, both gameplay and storywise,

For as many times as my squadmates jump out of the cover I put them in without me telling them to - directly in the line of fire...I'd have to say that's a terrible idea. Especially since squadmate AI drops dramatically on hardcore and insanity.

Though friendly fire should be a must. I mean, come on, if you're shooting at them, they should take damage. Though I'm not sure what should happen if their stupid butts shoot you in the back of the head...

Yay

Be better then ghost people immune to bullets. Why didn't they keep the smart ME1 shooting that didn't target squadmambers?

@ marshalleck - If you didn't like how ME2 played with Adepts...don't be an Adept. Lol. Besides, you should be able to have a well-rounded experience if you pick a bonus power that allows you to whittle down barriers, armor, and shields. Playing as an Infiltrator on hardcore was brutal with Harbinger until I switched my bonus power from armor-piercing ammo to Warp ammo...I had almost no way to whittle down his barrier quickly otherwise.

That achievement idea is a good one. I'll second it.

If the YMIR mechs are back then yay. Kill off my squad so they don't ruin the glorious headshot-explosion.
