Carver/Bethany: One dies in the prologue and one can die in the deep roads.
Anders: Murder knifed at the end.
Isabella: Tossed to the Arishok (although it is alluded to that she escapes).
Fenris: Tossed to Denarius as a footstool.
Sebastian: Throws a temper tantrum and runs to Starkhaven to raise an army against Kirkwall (yeah. right.)
So, actually Anders is the only one you can out-right kill.

Actually I do remember killing Fenris in combat at the end of one of my playthroughs.
I pretty well always knife Anders so I don't know if you can kill him in combat after that.
I didn't know about Merrill, but I would think that if everyone else is gone she would join you even if you side with the Templars.

If you let Anders go but side with the templars, he'll show up to fight you later on at the Gallows. Not sure about Merrill. I only had enough booze to get through the templar ending once and she didn't turn on me that time.
Hmm. Now that I think about it, both mages can turn if you side with the templars (no big surprise), both warriors can turn if you side with the mages (not surprised at Fenris, but I am surprised at Aveline), and both rogues stick with you no matter what. Odd that it's so well divided by class.

In short, what makes Merrill and Aveline so hard to betray you is that Merrill stays if Anders betrays you, and Aveline stays if Fenris betrays you. The only way to make Merrill betray you is to side with templars, however the moment you side with them, Anders is near 100% sure to betraying you, only by having him on high rivalry would he stay. Also, it is incredibly difficult to have Merrill's approval/hate be below 50. As for Aveline, the only way to have her betray you is to side with mages, the moment you do so, Fenris would turn against you, unless you have him on high approval/hate.

No. Treating him like an attack dog is confirming his prejudice beliefs that mages will treat others like pets or slaves. He is a person, and one that deserves nothing but contempt as far as I'm concerned. I have sympathy for his past and I can understand his distrust of mages but not to the extent that I'd forgive him for trying to kill someone that's always been a good friend to him for refusing to kill a group that includes his/her innocent sister.

Well Anders' rivalry Annulment ending is (apparently) bugged. So does that mean Merrill can't really side with the mages against Hawke until it's fixed?

If we lets say take Varric's talk with her as if they have met before or he is aware of her as granted.
That would still be quite a ret con to put her in the main game with Hawke contact.
As tbh.. the whole tell me the story, the true story spiel for someone with prior knowledge of it would just suck pink orbs of McGuffinhood.
Having the DLC post chapter three would be a little better... but again before Varric's meeting ould be silly.
Cassandra: "Oh yes the champion I had met and possibly "met", I thought it was another champion!."
... What... yes..!.
Only way to introduce her without nerdrage is after Varric and in an expansion for me.
Oh on topic: Aveline in her third act "questioning doubts" quest, I got her speaking with Hawkes voice on the last run!
