[Publius of NV]

This is from a game started with the 1.02 patch applied.  During Varric's "Haunted" quest, in the battle with the golem in the central room of Bartrand's Estate, there are two small benches near the stairs.  Shortly after the battle started, Isabela got stuck with one leg on one side of the right hand bench, and one leg on the other side.  I didn't see it happen, I'm guessing she did a back flip onto it. 

Anyway, once I noticed Isabela wasn't fighting anymore, I tried selecting her and ordering her to move - she couldn't.  I tried 4 different directions from that bench, and she was stuck there, could not move at all.  She was out of the battle completely and I had to finish it with Hawke, Varric, and Merrill.  Once the battle was over and the cut scene ended, she was back in the party, away from the bench.
