I don't know if this is a bug or what, but I've got the quest "Back alley justice"
and have successfully wiped out two of the maps.   I also have completed
"The Last Request", and was looking to finish the Back Alley quests....

However, I look on the Denerim city map, and I can't find the "Dark Alley"
map icon.   Is this a bug?

Yep, its a bug. Try to do the quest again but be sure to accept the quest that relate to Back Alley from the Chantry's Board first. Good luck
