As a tank you should wear the massive armor to improve the damage protection. Normally warrior is your first choice, another option is arcane warrior.
You should focus on strength and constitution, which will provide you high physical resistance. Strength will give you more damage output Constitution will provide you a lot of health, so you will survive longer in battle. (For arcane warrior, you should focus on magic and will power instead!)
You should have some item that help you to counter-spell or some talent that help you so (templar specialization, for example) to fight against the mage.
Some items and armor that I recommend to you:
- The life giver (give you a ton of health and health generation)
- Juggernaut armor set, provide you spell resistance, the same as ring of age.
- The Legion of the dead armor set, high armor and provide some good specs
- Wade superior dragon skin armor, provide you very high fire resistance and high armor.
About the weapon, that's up to you, there are ton of weapons that you can play with

Pretty good guide there, however slightly outdated.

You can make a tank 1 of 2 ways:

First: Max strength, 26 dex, base everything else. This tank will do pretty decent damage with a fullsize weapon (sword/axe) considering he is having to lug a shield around with him, and will also be quite survivable. Possible to out damage a two hander if built/geared/played right.

Second, around 36 Str, Max dex, base everything else. This gives amazing defence meaning you will hardly get hit, in massive armour when you do get hit you will take minimal damage, and you will need to use daggers. The Roses Thorn is the best dagger in the game, Duncan's Dagger, Dead Thaig Shanker and The Edge are also great.

If you have DLC's Cailen's armour set is excellent, Starfang/The Veshialle are the best 1 handed big weapons, best shields are Duncan's Shield, Fade Wall, and Cailen's Arm's are a great sword/shield set too.
The Spellward/The High Regard of House Dace are the best neck, Andruil's Blessing/Cinch of Skillful Maneuvering are best belts, Key to the City/Lifegiver are the best rings. However most cost a lot of gold.

Specs you should go Champion definitely, boost to attack and def is fantastic, and AoE knockdown is also great. Second spec is difficult to pick, Reaver isn't bad due to an aura that does damage every second to enemies in the radius coming out of the tank, however also does damage to the tank you need healer's on hand to keep you alive. Frightening Appearance is a good skill that causes enemies to cower in terror (basically stunning them and causing a lot of threat. It also makes Threaten/Taunt generate more threat.
Templar is also good. Has an Aoe stun/knockdown, cleanses area of harmful effects, and with the Templar exclusive Knight Commander's Plate you can achieve 100% spell resistance.

IMO you should never boost Con, 1 point in con gives 5 health, dexterity gives defence which is much more valuable. If you aren't getting hit you arent having to drink potions and your mages arent having to heal you, meaning you are able to pump out more damage as you arent having to stop. You should definitely boost your health with armour and trinkets, as you want to be able to survive Overpowers/Grabs by the more dangerous creatures.
Hope that helps!

What do you mean by max strength? :/ I always see people say to max something but I've never seen a cap for them. Just wondering what the "max" really is.

Max; as in just put points into strength, ie "just max out on strength" is shortened to "max strength". Hope that helps.

Ah so like I figured. No actual limit for them just dump points into it when you have them. :P
