I saw another thread - which I can't seem to find now that talked about this.  In act three Isabella appears in Hawke's mansion with a quest exclamation over her head but nothing happens when you try to talk to her.

I followed the Isabella FAQ but no go.  So far I've done:

1.  Spoke to her in the Hanged Man at the start of Act 3
2.  Completed "On the Loose"
3.  Have her Codex Entry "Isabella in the Last 3 Years"
4.  Gave her the Act 3 Gift.
5.  Completed "Best Served Cold" thinking that would help
6.  Running Patch 1.02

I'm out of ideas at this point so any help would be appreciated!  I suppose I could go back and restart Act 3 if that is a possible way to fix the bug.

Sometimes it helps to go away, do another character quest and go back.

Since you are on the PC, you can use the debug console to get around this and start the quest, though you miss the interaction she has at the very beginning of it. Enable the console, then runscript zz_isa_debug, select plot, select Act 3, select Jump to Point, then choose the Velasco encounter.

If you don't want to mess with the debug console, you can try restarting Act 3, or completing other quests. However, you may be stuck. The console is the only guaranteed way to get around it.
