I had it on Normal, but the boss on hard. I didn't realise this achievement until later.

So do I need to be on HARD mode from start to end, or just start and the end boss fight?

I heard of people on XBOX 360 that beat normal mode and than couldn't get the achievement afterwards. Anyone have that bug on the PC?

You need to have it from start to finish.

You should get it if you play the entire boss fight on Hard.

Did you start a new character with Witch Hunt or did you import? With a lv 35 warden, Witch Hunt should be a breeze on nightmare, especially since you have an actual healer mage, unlike with Golems of Amgarrak.
