I guessed as much (I remember getting the 3 flail head when I last played 10 years back). Not sure if it is a factor, but I am using baldurdash's fix (though not the official patch, which I cannot locate). I did give the bones to the 2 ghosts. The child ghost then appears and talks about giving me those items, yet they do not appear in my inventory (not too much of an issue, I just sleep off the duration of prot from undead, but still annoying in that I like to push my resources to the limits before resting).

I'm no expert, but I suspect this has something to do with it. I'm pretty sure your game will break horribly if you don't have to official patch before applying Baldurdash/G3 Fixpack.

[Ishad Nha]

A Protection from Magic scroll will protect a warrior from the Demi Lich spells. Kangaxx is easy to take down.
In Lich form hit him with Abi-Dalzim's Horrid Wilting.
In Demi-Lich form take him down with a warrior type protected with the Protection from Magic scroll. Have the warrior use the scroll as soon as he starts to turn into a Demi Lich.
