Blur is realistic. Just mostly over done in a lot of games. It doesn't annoy me. Film grain seems artificial but can give a nice stylistic effect. But sure, I accept that in real life i can't see super sharp textures (as it were) over unlimited distances. So depth of field and blur improve immersion for me.
Film grain and motion blur.. Do you use them?

Motion Blur: No
ME2

I don't on my SDTV, but on my 1080p one, they help take the edge off of the more jarring graphical imperfections.
