I just spent my 3 days of free time playing ME2 on insanity. I never changed the difficulty and beat the game continuing to my cabin. I have all the DLC missons and the only one i did was the shadow broker one and got the achievment for doing that on insanity. Soon and I got done with that i went throught the relay and beat the game didnt change anything.

To get the insanity achievement you do NOT have to do the entire game like ME1. You just need to do everything on the LEFT side of the Journal, DLCs are optional but not required, then survive the SM.
Start with a NEW character level 0, and not a NG+. I did it as a Sentinel with AP as a bonus because EVERYTHING has armor.
Pick which sidequests give you a research bonus that you want, damage, biotic, tech etc and skip all the rest.

It works on NG+ also. You just need to make sure you change the difficulty at the start screen before the opening cutscene kicks in.
