Does the NPC have a custom model equipped?
I recently saw something like this, which only became apparent when the NPC was walking several metres away from the main controlled character. The model seemed to warp/wobble alarmingly, with the same rhythm as the walking motion.
The problem went away when I substituted a Bioware model.
There is a different issue when the walker is out of sight. Even if AMBIENT_SYSTEM_ALWAYSON is set, and the AI level is very high, the engine still seems to suspend walking. When the main controlled character approaches the walker again, they remain stationary for a moment before resuming movement. However, I haven't seen any warping in that situation.

It does loook like custom model. And that could well be the reason, then -- some of custom models only provide the highest level of detail, the _0 mesh. The game engine tries to replace that with _2 mesh when the character is farther away. If this _2 mesh is missing, then the end result is the _0 mesh being animated with much simplified level _2 skeleton. Which creates the warped appearance like on the screenshot.
