Ok, ive always picked the flirt options, gave her the knife and completed the 'mirror image' and 'back from Sundermount' quests but i havent got the 'questioning beliefs' quest and ive just done the quest where Izzy leaves so ive not got much of act 2 left...
what the hell have i done wrong here? I dont think ive missed a step...ive even given her the first gift

You have to get the 'Questioning Beliefs' quest before the romance advances. Do you have more than 50 % friendship/rivalry? If you do, it should trigger. If not... a bug, maybe?

well, her friendship is into the blue part...not sure how much, but ive flirted everytime ive talked so I understood that should be enough...i wish it had a damn number like DAO...then i can tell how much it is
I know you can romance them without them being in your party, just not sure what ive missed...very annoying

Yeah, I sympathize. All I can suggest is taking her on all the mage/demon related quests and side with mages/listen to the deals demons want to make in order to raise her friendship and be nice to her, or side against mages/kill demons on sight in order to gain rivalry.
However. If you don't want to do all those quests over again, an easy way would be to replay her personal quest and refuse her the arulinholm. This will immediately set her to 75 % rivalry, I think, and you'll gain a lot of rivalry in the talks with her afterwards. If you're willing to do the rivalry romance, that is - it isn't badly done at all.

Effectively you need her bar to be at 50% into either the blue side, or the red side. If her specialty bonus (Either the friendship or the rivalry bonus) is activated, that means you're 50% or beyond and her QB will trigger. If its not, then go back to a previous save and drag her along for every pro-mage quest, every pro-demon quest (aka taking the time to listen to the demon rather then outright denying) until it turns on.
