I am having the same problem and I am also wondering if it has to do with buying ingredients from the Black Emporium.
I tried that yesterday to see, I went to the Black Emporium and bought all the ingredients they had. I didn't get the achievement. Either you have to start a whole new game for it to work or it's something else, I am not convinced all the ingredients you failed to pick up are available at the black emporium. I only had saves after the final battle so I can't be 100% sure either way.

Well,
when I reloaded a save where I hadn't used a Maker's Sigh and Isabela's
outfit showed up when I loaded the save after the patch whereas before the patch I had to use a maker's sigh to get it to appear. I found out that even if the romance doesn't continue, Isabela's outfit updates just for having slept with her.
I started a whole new game so I'll have to see if after I bed her that her outfit updates.

mage still spins staff, warriors spin or pump with their weapon, havent tried a rogue with a bow however, but i am assuming the same thing occurs, that the sound triggers but not the action, if anyone could clarify that, would help in finding the problem i would assume.
edit: sebastian at the least twirls his bow still, so maybe its just a dual weapon rogue thing?

Yes, he should show up as dead, meaning you wont get his quest. That was fixed before this patch actually.
is it retroactive? I still had problems my two Zevran-dead imports even after patch 1.01 so thats why I've been so confused.
I would assume it isnt then, because on my zevran dead import it worked correctly. So you would need to start a new game.

This is the codex-in-question I lifted from the DA wiki:

Any word the devs could give me on whether or not the codex issue will be addressed in the next tune-up?

My rogue archer still twirls her bow, so that seems to be working ok (at least for me).

It sucks because I get those last two items after my save and the achievement doesn't pop and I have all 66 items. Why can't the game see I have those items and give me the achievement? It's crap.

I've just replayed the last couple of hours to re-order the romance until after I'd done the first two Qunari missions in the hope that the Friendly concern conversation would trigger (it still hasn't, boo).
Since I find it odd when a conversation cut scene starts with an active party member suddenly mid-conversation with some-one else, I cleared my party, spoke to Isabela in the Hanged Man after the romance, and have just noticed that by doing this then re-adding her, her romance outfit updates.
It didn't change straight away though - only when I left the Hanged Man and went straight to Merrill's house - upon loading it had updated.
Coding gremlins at work?

repeatedly completing a single side quest.
Aww. I guess I won't download it then, lol. I don't like the new leveling system. I can't get a semi start till I force level up. The talents nickel and dime hardcore for just basic stuff and with even longer cooldowns, u need more talents. Grrr. Screw that.

I killed him 3 times in 3 of my imports post patch 1.02 and he still lives.
I thought this bug was fixed
