So I was reading the guide:

And I've done the prequesite (got her up to 80 approval, "Warm"), but it refuses to give me random encounters where she gets wounded or whatever. I've travelled around several times with her and it won't do it. Is it possible I already fought "Low Road" and "Hillside Path" before she even joined my party? (I definitely recall quite a few random encounters with darkspawn...)

2. Double check. More might have appeared.
3. Does she have Vessel of the Spirit as a skill?
You shouldn't have to worry about missing the random encounter, since it only triggers when she's in the party.

The required encounter doesn't always happen right away. Once you have the necessary approval, just give it time. I've had this encounter sometimes happen early on, and sometimes right before the Landsmeet.

To get her quest, first boost her approval to be high enough, and then, try to finish some conversation with her. Her quest only appear after several conversation, I can confirm that

I've gotten her companion quest before her Approval was at 80, so I'm fairly certain that it has nothing at all to do with her Approval level. (Or at least, nowhere near that high. You may need a minimum of 25, since that appears to be the level for several other companions like Leliana and Sten to start opening up more to you.) Instead, I believe it triggers once you've had one (possibly several) conversations with her. In particular, the ones relating to abominations and whether they can be saved. That will activate the random encounter where she falls unconscious, and after that, a second encounter where she learns her Vessel of the Spirit power.
After you've had those encounters, you now have an option to talk to Wynne about 'regrets', and this is the conversation that triggers her companion quest.
